**Advertised Summary Job Description**

The Clinical Coordinator - NP will guide patients through the Oncology care system, establishing contact with newly diagnosed cancer patients and/or family member, or caregiver. S/he will understand the clinical pathways established by the CUMC Oncology Program and will direct the patients to health care services within the organization, at outside facilities, and within the community for timely diagnosis, treatment and survivorship. S/he will actively identify and address barriers to care that might keep the patient from receiving timely and appropriate treatment for their cancer diagnosis. S/he will connect patients with resources, health care and support services in their communities and assist the patient and oncology physicians in the transition from active treatment to survivorship.

The Clinical Coordinator - NP will work closely with the CUMC Oncology Program and community based service providers to identify and address community needs for cancer screening services and education regarding cancer prevention and early detection. Will be responsible for ongoing evaluation and quality improvement of the Navigator Program and will track all activity as defined by the Oncology Program.

**Minimum Qualifications for Grade**

Requires a Bachelor's degree or equivalent in education and experience, plus four (5) years of related experience. Degree in Nursing, BSN, and Nurse Practitioner license required.

Must be detailed oriented and work well in groups. Excellent oral, written and interpersonal skills.
